We can issue Family Register Certificates for those whose registered domicile is in Niseko Town, and Resident Certificates (Juminhyo) for those whose resident registration is in Niseko Town.
From March 1, 2024, even if your registered domicile is far away, you will be able to request a Family Register Certificate at your nearest municipal office.

1. Those who can make a Request

2. Required Documents
When the person in 1(A) makes a request
1. Identification document of the person coming to the counter (driver's license, health insurance card, My Number Card, etc.)
2. If the requester's name is not listed on the family register when the request is made by a direct relative (For example, when a child leaves their parents' family register through marriage and a new family register is created for the couple, and the child requests a certified copy of their parents' family register) we ask for documents that can confirm that the requester is a direct relative of the person listed in the family register (certified copy of family register, etc.)
3.If the request is made by a representative, a power of attorney is necessary.
When a person in 1 (B) to (D) makes a request
1. Identification of the person coming to the counter (driver's license, health insurance card, My Number Card, etc.)
2. In the case of a request from a representative of 1(B)-(D), a power of attorney prepared by 1(B)-(D)

If you live far away or are unable to come to the office during the day, you can also apply by post.

2. Copy of identity verification documents
A copy of your Residence Card, driver's license, health insurance card, My Number Card, etc.
3. Documents that show the relationship with the intended person
If the requester's name is not listed in the family register, documents that can be used to confirm that the requester is a direct relative of the person listed in the family register (certified copy of family register, etc.)
4. Fee
Please purchase a "fixed amount credit exchange" (Teikaku Kogawase Shosho) for the necessary amount at Yucho Bank. Please enclose the certificate without writing anything on its front page.
5. Self-addressed envelope and stamps
・Please fill in the name and address of the requester listed in "(1) Application Form" as the address on a self-addressed envelope. Please stamp with sufficient postage.
・Certificates will be A4 or B4 size, so please avoid small envelopes.
・If you are sending a large number of documents, please send them without printing the stamps on the envelope (Put the stamps into the envelop. We will return all unused stamps to you), or write in red ink on the return envelope, "Deficit payment by recipient."
