Birth notification (after your child is born)

Please submit the birth notification at the town hall within 14 days after childbirth.
Eligible PersonsFather or mother of the child
Notification PlaceAt the municipal office of either the notifier’s current address, location of Family Register (only for Japanese citizen), or the newborn’s place of birth
Required Documents· Birth Notification Form (must be completed by a doctor or midwife)
· Maternal and Child Health Handbook
· The child's health insurance card
Other・People with the National Health Insurance will receive a one-time childbirth allowance of 500,000 yen.
· For children's medical care and child allowance, additional procedures are required.

Death notification

Please complete the necessary procedures within seven days from the date on which you became aware of the death.
Eligible PersonsRelatives, cohabitant, etc.
Notification PlaceAt the municipal office of either the deceased person's place of death, the location of Family Register (only for Japanese citizen) or the persons current address.
Required Documents・Death Notification Form (also available at the town hall.)
・Death Certificate (must be certified by a doctor.)
・ Niseko Town National Health Insurance Card (insured members only.)
OtherIf the person who passes away is insured by the National Health Insurance, funeral expenses of 30,000 yen will be provided.

Marriage Notification

The legal effect will begin from the date the notification is submitted.
Eligible PersonsHusband and/or wife.
Notification PlaceAt the municipal office of either the notifier's location of Family Register (only for Japanese citizen) or current adress.
Required Documents· Marriage Notification Form
・Identification card of the person submitting the notification (driver's license, My Number Card, etc.)
Other· Minors must obtain consent from their parents.
・Those who move into Niseko Town due to marriage must submit a moving-out certificate issued by the previous address's municipal office.
Moving within the town also requires a separate procedure from registering your marriage.

Divorce Notification

The legal effect will begin from the date the notification is submitted.
Note: In the case of a court-ordered divorce, please complete the procedure within 10 days after the final judgement.
Eligible PersonsHusband and/or wife (or complainant in the case of judicial divorce.)
Notification PlaceAt the municipal office of either the couple"s location of Family Register (only for Japanese citizen) or current adress.
Required Documents· Divorce Notification Form (In the case of a divorce by mutual consent, the signatures and seals of two adult witnesses are required.)
· In case of divorce by mediation: A certified copy of the mediation record.
· In case of divorce by arbitration or litigation: A certified copy of the arbitration transcript, or a certificate of judgement.)
