Tax Certification Application and Notification Forms

Various types of tax certificates are handled by the Tax Division. If you need a certificate, you can either visit the Tax Division counter in person or download the application form (below) for tax certificates and mail it to us.

Applicants are required to provide identification when applying in person or by mail.

How to request various tax certificates by mail

Please prepare the following documents and mail them to Niseko Town Town Hall Tax Division. 
InvoicePlease fill in the necessary information on the application form for issuance of tax certification, etc. or letter paper.

Vehicle number (if tax payment certificate for light vehicle inspection is required)
Location of land and house (if a certificate related to property tax is required)
Copy of identity verification documentA copy of the requester's (or representative 's representative if a power of attorney is attached) driver's license, health insurance card, My Number Card, or other document that can be used to verify the requester's identity
Reply envelopeAffix a stamp and write the address
Fixed amount postal money order for commissionPlease purchase it at the post office and send it without writing anything.
*Please do not give change.
*Depending on the purpose of use, fees may be waived (special disease medical treatment, child (support) allowance, medical assistance, etc.). If you have any questions about whether your purpose of use qualifies for an exemption, please contact Tax Division.
Letter of attorneyThis is required when requesting a certificate for someone other than the applicant (not required if a relative living with the applicant is requesting on behalf of the applicant).
You can use the Power of Attorney field on the application form for the issuance of tax certificates, or you can use any format.
Other・If the heir makes a request, a copy of the family register that can confirm the start of inheritance (death of the inheritee) and a copy of the document that proves the heir (family register, etc.) is required.
・Please note that if you pay the town tax at a convenience store or a smartphone app, it will take time to issue a tax payment certificate.
