Application procedure
If Resident Certificate address is in Niseko Town and you receive the vaccination in another Municipal
No notification is required to Niseko Town.

If you have Resident Certificate outside of Niseko Town and are receiving the vaccination in Niseko Town
If you meet the requirements 1 to 5, please complete the necessary procedures at the Niseko Town Hall counter (Health & Welfare Division, Infectious Disease Control Section).
You will need the Resident Certificate vaccination voucher issued by your registered address, so please bring it with you.
