Vaccination certificate (vaccine passport)

Those who have been vaccinated in Japan using the vaccination voucher issued by Niseko Town
*Those who had Resident Certificate in Niseko Town at the time of vaccination

You can apply using the dedicated app on your smartphone. Please see below for details on how to download the app and how to apply.

[For overseas and domestic use]
(1) My Number Card + 4-digit PIN
(2) Passport valid for overseas travel * 1
[For Japan]
(1) My Number Card + 4-digit PIN
 
Note: Your PIN is a four-digit number (used to assist you in entering information on the card) that you set when you received My Number Card at your local municipal office.
Note: In the case of electronic delivery, the original is required because (2) above is read by a smartphone.
 
* 1 The passport number printed on the vaccination certificate and the passport number used for overseas travel must match. If your passport number changes after you have obtained your vaccination certificate, you will need to obtain your vaccination certificate again. If you are applying for a passport, please start applying for a vaccination certificate after your passport is issued. You can also apply with a passport issued by a foreign government, such as a foreign national.

Applications must be made at the Niseko Town Town Hall counter (Health & Welfare Division Infectious Disease Control Section).
Usually, it will be issued the day after the application.

(2) Identity verification documents (at least the name and date of birth are stated)
(3) Certificate of inoculation
 
Note: In the case of delivery in writing, both (2) and (3) above may be copied.

Applications can be made by mailing the necessary documents to Niseko Town Hall Health & Welfare Division Infectious Disease Control Section.
Normally, the required documents will be delivered and returned after the next opening day after the documents are received at the government office.
(Please consider that you will receive your vaccine passport 1-2 weeks after mailing the required documents.)
