2回目接種日から5か月が経過した方。

2回目接種日から5か月が経過する方に送付しています。

* If you move to Niseko Town after completing two vaccinations, you may not receive the vaccination voucher. Applicable people need to go through the procedure for issuing an vaccination voucher at the office. Please complete the procedure at the Health and Welfare Division with the records of the first and second vaccinations and documents that can confirm your identity.
