Received two new coronavirus vaccinations,
2回目接種日から5か月が経過した方。

2回目接種日から5か月が経過する方に送付しています。

*If you have received both vaccinations and then moved to Niseko Town, you may not receive your vaccination voucher. If this applies to you, you will need to apply for vaccination voucher at the town hall counter. Please bring your first and second vaccination records and documents that can verify your identity to Health & Welfare Division to apply.
