Vaccinations

We will provide a subsidy of 3,000 yen per dose for the following people who are vaccinated against pneumococcal at Niseko Clinic or a family medical institution outside the town. It is a one-time grant per person.

5歳刻みで対象年齢を設定している経過措置は令和5年度で終了となり、令和6年4月1日以降は、以下の方が定期接種の対象となります。
（1）接種日現在で満65歳の方
（2）接種日現在で満60～64歳の方で、心臓、じん臓、呼吸器の機能、又はヒト免疫不全ウイルスにより障害のある方

• 接種費用のうち3,000円を助成します。
• Those on public welfare will be free of charge. (Procedures must be completed in advance at the municipal office.)

令和7年3月31日まで
Please inoculate within the period as it will not be eligible for subsidy after the period covered above.
Those who have been vaccinated at a medical institution outside the town will be reimbursed by presenting a receipt, so please complete the procedures at the Health Promotion Section of the Health and Welfare Division.

Get a flu shot at Niseko Clinic or your family medical institution outside town

• Those 65 years old and over
• Those who are between the ages of 19 and 64 and who belong to household tax exemption / livelihood protection households (In case of receiving at Niseko clinic, proceedings are required at the town hall in advance)
• Those who are from 6 months old to 18 years old (high school student equivalent age)

For the target, we support up to 3,400 yen per inoculation.

• ニセコ町インフルエンザ予防接種費用助成申請書（役場で記入、または下記よりダウンロードして持参）

ニセコ町内にお住まいの接種日時点で65歳以上の方 で、令和5年（2023年）4月1日以降に帯状疱疹予防接種を受ける方。

• 不活化ワクチン（乾燥組替え帯状疱疹ワクチン シングリックス）
• 生ワクチン（乾燥弱毒生水痘ワクチン ビケン）

1回につき5,000円（不活化ワクチンは2回、生ワクチンは1回を上限に助成します。）
ただし、生活保護法による保護を受けている方は、無料です。

○ニセコ医院で接種する場合
1.ニセコ医院に予約をします。
2.帯状疱疹予防接種申請書・予診票を受け取って、記入します。
3.接種当日は、保険証や運転免許証などの身分証明書類と記入済みの申請書・予診票を提出します。
4.会計にて、町からの助成費用5,000円を差し引いた額（不活化ワクチンの場合 17,000円、生ワクチンの場合 2,230円 をお支払いください。）
（不活化ワクチンの場合は、2回目も同様の流れとなります。）
 
※ 生活保護法による保護を受けている方は、ニセコ医院でのみ接種が可能です。なお、ニセコ医院に予約をする前に、役場で手続きが必要です。

○町外医療機関で接種する場合
1.希望する医療機関に予約をします。その後、医療機関の指示に従い、予診票を記入してください。
2.会計にて、接種費用の全額をお支払いください。その際に、領収書（帯状疱疹予防接種を受けたことがわかる内訳書含む）を受け取ってください。
3.ニセコ町役場保健福祉課窓口にて、帯状疱疹予防接種費用助成申請書を記入します。（または、下記よりダウンロードして持参）
【持ち物】・医療機関が発行した領収書（帯状疱疹予防接種を受けたことがわかる内訳書含む）
　　　　　・振込先の口座がわかる通帳 又は キャッシュカード（口座名義が申請者と同じもの）
4.後日、指定の口座に助成費用を償還払いします。
