Maternal and Child Health Handbook

　妊娠したことが分かりましたら、妊娠届出書に必要事項を記入の上、すみやかに保健福祉課へ提出してください。
（妊娠届出書は受診した医療機関で発行されます）

• Pregnancy notification form etc. issued by the hospital
• 妊婦本人のマイナンバーカードまたは運転免許証
• Personal seal (Inkan)
• 振込口座確認書類（申請者ご本人の通帳、またはキャッシュカードの写し）※妊婦のための支援給付金で使用します

A consultuation with a public health nurse and dietitian will be held when the Maternal and Child Health Handbook is issued.

　母子健康手帳と一緒に「妊婦一般健康診査受診票」14回分、「超音波検査受診票」10回分をお渡しします。
ニセコ町へ転入される方は転入手続きの際にお申し出ください。
町外へ転出するとニセコ町発行の受診票は使用できませんので、転出先の市町村で受診票の発行をしていただきます。

～北海道外で妊婦検診等を受ける場合～
　受診票は北海道内の医療機関のみで使用できます。里帰り分娩などで道外の医療機関を受診する場合は、医療機関で一旦全額をお支払いいただき、償還払いを申請していただくことにより妊婦検診費用の一部を助成します。（海外の医療機関は除く。）該当する方は事前にお問い合わせください。

The program is held as a place to nurture the desire to become a parent and family bonding while learning about pregnancy, childbirth, and childcare.
