Maternal and Child Health Handbook

Issuance of Maternal and Child Health Handbook

Maternal and child health handbooks are issued to pregnant women.
Please notify us as soon as possible in order to receive the handbook, as it is used to record your pregnancy and child's growth.

Necessary Items

  • Pregnancy notification form etc. issued by the hospital
  • The pregnant woman's My Number Card or My Number notification card
  • Personal seal (hanko)
  • Bank account verification document (copy of applicant's bankbook or cash card) *Used for transfering childbirth support benefits

Remarks / Other

A consultuation with a public health nurse and dietitian will be held when the Maternal and Child Health Handbook is issued.

Pregnancy Health Check-up

We issue 14 General Prenatal Health Check-up Coupons and 6 Ultrasound Examination Coupons for all pregnant women (resident of Niseko Town). These coupons will be handed out at the same time the Maternal and Child Health Handbook is issued.
  • These coupons are only valid for medical institutions in Hokkaido. If you are planning to have pregnancy health checkup/childbirth at a medical institution outside of Hokkaido, due to returning to your hometown or such, please contact us in advance of your departure. Please also contact us if you have already left.
  • If you're pregnant and moving into Niseko, please let us know when completing your move-in procedures.
  • If you want to move-out of town, please contact the person in charge of pregnancy health checkups.

Papa-Mama Seminar

The program is held as a place to nurture the desire to become a parent and family bonding while learning about pregnancy, childbirth, and childcare.
