What to do if you do not receive vaccination voucher for the third dose of the COVID-19 vaccine

For those who have not received vaccination voucher for the third dose of the COVID-19 vaccine
For those who have completed their second vaccination by September 21, 2021, 2021 will receive the new coronavirus vaccine vaccination voucher for the third vaccination and vaccination information for April.
If you received your vaccination outside of Niseko Town and have completed your vaccination during this period but have not received vaccination voucher,Submit documents such as "New Corona Vaccination Record" to the Niseko Town Hall to show that the two vaccinations have been completed.and receive vaccination voucher.
One of the reasons why vaccination voucher have not arrived is that the medical institutions that administered the vaccinations have not registered the vaccination records in the national registration system (VRS), so municipalities are unable to obtain vaccination data from the national government. One possible reason is that vaccinations were administered without using vaccination voucher for the first and second doses, such as in workplace vaccinations, and therefore the ticket number is unknown and registration is not possible.
*If you still have vaccination voucher for the first and second doses, please submit them to the medical institution where you received the vaccination.

People who moved to Niseko Town after January this year may not have received vaccination voucher has not been received in time, so if this applies to you, please contact the Infectious Disease Control Section of Health & Welfare Division.
