About establishment of application site for sustaining benefit application
Notice
Regarding the application of sustainable benefit, the assistant will support the input of electronic application at the application support venue only for those who do not know how to apply electronically. Along with this, we will open four support venues in Shiribeshi.

Terms of use
1.Reservation is required in advance.
In order to avoid the spread of infection, advance visits are required to use the application support venue. Please make a reservation first and visit us. Reservations can be made by either of the following telephones.

Reception hours: 24 hours can be reserved * The venue number of the venue to be reserved is required.
・ Application support venue telephone reservation window (operator correspondence)
Phone number: 0570-077-866
Reception hours: 9am to 6pm daily
2. Please bring your application assistance sheet.
As preparation, please fill in the "Application Assistance Sheet" with the information required for application and bring it with you on the day. (See link below)
3.Please prepare and bring the necessary documents
Please be sure to bring the necessary documents with you when you apply.
There is no copy machine at the application venue. In addition, you cannot receive the data with a USB memory, so please bring a paper copy or a copy.
Please note that the required documents differ for "small and medium-sized corporations" and "individual businesses".
Also, please bring a writing implement such as a ballpoint pen on the day as a measure against the new corona.
